Momentum For Mental Health

Momentum’s Youth and Family Services department offers comprehensive mental health services to over 200 at-risk youth, ages 10 to 25, annually. The Transitional Age Youth (TAY) and Raising Early Awareness and Creating Hope (REACH) programs, within the department, provide outpatient mental health services, group therapy and skill development to youth in addition to educational support, counseling, mentoring, vocational services, a safe place for self-exploration, and access to community resources.

Momentum’s Youth and Family Services program also emphasizes early intervention and prevention by informing the public about the warning signs of psychosis and treating youth before a mental illness progresses and becomes more difficult to manage. This also enables Momentum to positively influence individuals at an early age and remove the perception that mental illness, family hardships or other current circumstances dictate their futures.

Momentum was founded in 1997 when four respected agencies merged together to provide better care and access to mental health treatment in the community. Today, it is the largest private nonprofit provider of youth and adult mental health services in Santa Clara County.
